The request must include the following details: a) Full name; b) Original or authenticated copy of their National Identity Document (DNI) or an equivalent legally valid document that proves their identity; c) Address; d) Postal Code; e) Telephone number; f) Email; g) Specification of the type of right being exercised(e.g.,right of access, deletion, rectification, updating or submission to confidentiality). In these last three cases they must provide a brief explanation justifying the request; h) Indication of the preferred means for receiving the information, which may be provided in writing, by electronic, telephone, image, or any other suitable means,a as per article 15.3 of Law 25.326;and i) An holographic signature at the end of the application.
